Crossroads: For Young Adults

Crossroads helps young adults ages 17.5 to 22 who have a history of serious emotional disturbance or a current serious mental illness and are likely to benefit from a continuum of mental health services that are community-based rather than clinic-based.

Crossroads staff work with young adults to set personal goals - vocational, housing, educational, and social goals – develop plans to achieve those goals, and help them to access supportive services.

Crossroads’ multi-disciplined team of licensed clinicians, case managers, and psychiatrist help these young adults move toward self-sufficiency and economic security by providing a full range of mental health treatment and case management services.  Case management staff meet with individuals and their families at locations of their choice, such as in their homes, at school and at other community settings.

Crossroads staff collaborate with service providers such as schools, substance abuse treatment services, residential treatment facilities and hospitals, private providers of mental health services, and multiple State and County agencies including Westchester County Departments of Community Mental Health, Social Services, and Adult Protective Services.
