Children's Case Management Services(CCMS)

The goal of case management services is to provide individualized, community-based care coordination to children and their families, where a serious emotional disturbance on the part of the child threatens a family’s ability to keep the child at home. CCMS programs include the Home and Community Based Services Waiver, Children's Intensive Case Management and a non-billable case management program.

Children’s Case Management Services Care Coordinators work with parents and other community service providers using a strength-based model which identifies a support team for each family and which advocates for needed and appropriate services to promote the successful care of children within their homes and communities.

Are you a parent/caretaker of a child or adolescent with documented serious behavioral or emotional difficulties, which have led to psychiatric hospitalization or put the child at risk of residential placement? Are you willing to work with a care coordinator, in your home, to explore available services, develop respite options and learn new parenting strategies?

Are you a mental health, school or other human services professional working with a seriously emotionally disturbed child who needs additional support to avoid re-hospitalization or placement?

What We Deliver

Specially trained Care Coordinators experienced in working with emotionally disturbed children meet regularly with children and families in their homes to develop service plans, identify community supports and advocate for needed supports. Collaboration and cross systems planning with Special Education, schools, mental health treatment providers, probation, Department of Social Services, respite and recreation are essential features of Children’s Case Management.

For families with youth at risk of residential placement due to court involvement, MHA offers specialized services in the Partnership program.

Care Coordinators are based in Elmsford but provide community-based services throughout Westchester County in the homes and communities of the families.

When

Care Coordinators provide at least weekly visits in addition to collaboration with additional service providers. Emergency beeper coverage is available after hours, weekends and holidays with immediate access to a member of the program team.

Languages Spoken

Intensive Case Management and Non-Billable Case Management Services are available in Spanish and English.

Cost

There is no cost to a family whose child meets the eligibility requirements for Case Management Services - documented serious behavioral or emotional difficulties which have led to psychiatric hospitalization or which put the child at risk of residential placement. Medicaid will be billed, if available.

Staff/Service Providers

Children’s Case Management Services are staffed by professional social workers. Staff members have considerable knowledge about the Children’s Mental Health Service System, including treatment, family support, residential placement options and process, educational classification and placement and Community Service Networks.
