With a few minutes of your time, you can help Westchester County address the significant mental health needs of our community. The Westchester County Department of Health is conducting a county-wide survey that will help to better understand how the COVID-19 pandemic has impacted the health and well-being of residents. The anonymous survey is crucial to inform health policies and initiatives that promote change and address social determinants of health within our community, and gives individuals, like you, the opportunity to express what matters to them.

What is the purpose of this survey?
The answers from the survey will help the Westchester County Department of Health assess the current availability and accessibility of health care services, and identify health priorities in the county.

How will the survey benefit the community?
The data collected will help the county set goals, plan and implement programs that address disparities in care and promote physical and mental wellbeing for all residents. 

Who can fill out the survey?
The survey is open to all Westchester county residents who are 18 or older.

How long will the survey take?
The survey will take approximately 10 minutes to complete.
